ABSTRACT Introduction: Cardiac surgery-associated acute kidney injury (CSA-AKI) is a powerful predictor of perioperative outcomes. We evaluated the burden of CSA-AKI in patients with preserved baseline renal function. Methods: The data of 2,162 adult patients who underwent cardiac surgery from January 2005 to December 2020 were analyzed. Logistic regression models were used to determine predictors of CSA-AKI and their associations with hospital mortality up to 30 days. Results: The prevalence of acute kidney injury was 43.0%, and 2.0% of patients required renal replacement therapy. Hospital mortality rate was 5.6% (non-acute kidney injury = 2.0% vs. CSA-AKI = 10.4%, P<0.001), and any degree of CSA-AKI was associated with a significant increase in death rates (stage 1 = 4.3%, stage 2 = 23.9%, stage 3 = 59.7%). Multivariable logistic regression analysis identified age, obesity, left ventricular dysfunction, previous cardiac surgery, and cardiopulmonary bypass duration as predictors of CSA-AKI. Moreover, CSA-AKI was confirmed as independent predictor of hospital mortality for stage 1 (odds ratio, 2.02; 95% confidence interval, 1.16 to 3.51; P=0.013), stage 2 (odds ratio, 9.18; 95% confidence interval, 4.54 to 18.58; P<0.001), and stage 3 (odds ratio, 37.72; 95% confidence interval, 18.87 to 75.40; P<0.001) patients. Conclusion: Age, obesity, left ventricular dysfunction, previous cardiac surgery, and cardiopulmonary bypass duration are independent predictors of CSA-AKI in patients with preserved baseline renal function. The development of CSA-AKI is significantly associated with worse outcomes, and there is a dose-response relationship between acute kidney injury stages and hospital mortality.

Abstract Background: Renal dysfunction is an independent predictor of morbidity and mortality in cardiac surgery. For a better assessment of renal function, calculation of creatinine clearance (CC) may be necessary. Objective: To objectively evaluate whether CC is a better risk predictor than serum creatinine (SC) in patients undergoing cardiac surgery. Methods: Analysis of 3,285 patients registered in a prospective, consecutive and mandatory manner in the Sao Paulo Registry of Cardiovascular Surgery (REPLICCAR) between November 2013 and January 2015. Values of SC, CC (Cockcroft-Gault) and EuroSCORE II were obtained. Association analysis of SC and CC with morbidity and mortality was performed by calibration and discrimination tests. Independent multivariate models with SC and CC were generated by multiple logistic regression to predict morbidity and mortality following cardiac surgery. Results: Despite the association between SC and mortality, it did not calibrate properly the risk groups. There was an association between CC and mortality with good calibration of risk groups. In mortality risk prediction, SC was uncalibrated with values > 1.35 mg /dL (p < 0.001). The ROC curve showed that CC is better than SC in predicting both morbidity and mortality risk. In the multivariate model without CC, SC was the only predictor of morbidity, whereas in the model without SC, CC was not only a mortality predictor, but also the only morbidity predictor. Conclusion: Compared with SC, CC is a better parameter of renal function in risk stratification of patients undergoing cardiac surgery.


Resumo Fundamentos: Disfunção renal é preditor independente de morbimortalidade após cirurgia cardíaca. Para uma melhor avaliação da função renal, o cálculo do clearance de creatinina (CC) pode ser necessário. Objetivo: Avaliar objetivamente se o CC é melhor que a creatinina sérica (CS) para predizer risco nos pacientes submetidos à cirurgia cardíaca. Métodos: Análise em 3285 pacientes do Registro Paulista de Cirurgia Cardiovascular (REPLICCAR) incluídos de forma prospectiva, consecutiva e mandatória entre novembro de 2013 e janeiro de 2015. Foram obtidos valores de CS, CC (Cockcroft-Gault) e do EuroSCORE II. Análise de associações da CS e do CC com morbimortalidade foi realizada mediante testes de calibração e discriminação. Por regressão logística múltipla, foram criados modelos multivariados independentes com CS e com CC para predição de risco de morbimortalidade após cirurgia cardíaca. Resultados: Apesar da associação entre a CS e morbimortalidade, essa não calibrou adequadamente os grupos de risco. Houve associação entre o CC e morbimortalidade com boa calibração dos grupos de risco. Na predição do risco de mortalidade, a CS ficou descalibrada com valores >1,35 mg/dL (p < 0,001). A curva ROC revelou que o CC é superior à CS na predição de risco de morbimortalidade. No modelo multivariado sem CC, a CS foi a única preditora de morbidade, enquanto que no modelo sem a CS, o CC foi preditor de mortalidade e o único preditor de morbidade. Conclusão: Para avaliação da função renal, o CC é superior que a CS na estratificação de risco dos pacientes submetidos a cirurgia cardíaca.

Abstract Objective: We evaluated whether the preoperative serum concentration of brain natriuretic peptide (BNP) is a predictor of in-hospital mortality in patients that underwent cardiac surgery. Methods: We continuously evaluated 488 patients that underwent cardiac valve surgery or coronary artery bypass grafting (CABG) between January of 2009 and July of 2012. Follow up of these patients were done prospectively for 30 days postoperatively. Results: Data analysis showed that the overall mortality rate was equal to 9.6%, Receiver Operating Charactheristic (ROC) curve analysis found the optimal cut-off value of BNP equal to 382 pg/mL for overall mortality (AUC=0.73, 95% CI=0.66 to 0.81, P<0.001). Multivariate analysis showed that the value of BNP higher than 382 pg/mL (P=0.033, HR=2.05, 95% CI=1.6 to 3.98) was an independent predictor of overall mortality at 30 days postoperatively. Conclusion: We concluded that the preoperative serum concentration of BNP is an independent predictor of mortality in patients undergoing valve surgery or coronary artery bypass graft. .


Resumo Objetivo: Avaliar se a concentração sérica pré-operatória de peptídeo natriurético cerebral tipo B (BNP) é preditora de mortalidade intra-hospitalar em pacientes submetidos à cirurgia cardíaca. Métodos: Foram avaliados 488 pacientes consecutivamente submetidos à cirurgia cardíaca valvar ou à cirurgia de revascularização do miocárdio no período de janeiro de 2009 a julho de 2012. Estes foram seguidos, prospectivamente, por 30 dias de pós-operatório. Resultados: Em nossa casuística, a mortalidade geral foi igual a 9,6% e 52% dos pacientes foram submetidos a cirurgia de revascularização do miocárdio. Análise de curva ROC (Receiver Operating Charactheristic) encontrou o valor de corte ótimo de BNP igual a 382 pg/mL para mortalidade geral (AUC=0,73, IC95%=0.66 a 0,81, P<0.001). Análise multivariada mostrou que o valor de BNP > 382 pg/mL (P=0,033, HR=2,05, IC 95%=1,06 a 3,98) foi preditor independente de mortalidade geral em 30 dias de pós-operatório. Conclusão: A concentração sérica pré-operatória do BNP é um preditor independente de mortalidade em pacientes submetidos à cirurgia valvar ou de revascularização do miocárdio. .

Fundamentos: Indicadores são ferramentas úteis para se conhecer o perfil da prescrição. Objetivo: Analisar os indicadores de prescrição medicamentosa recomendados pela Organização Mundial da Saúde em prescrições de pacientes idosos internados em enfermaria de cardiologia do sistema público de saúde. Métodos: Estudo exploratório-descritivo de abordagem qualitativa através da análise de 1382 prescrições, durante o período de internação, de 223 pacientes idosos admitidos na enfermaria da cardiologia clínica de um hospital de ensino. Resultados: A média de medicamentos por prescrição foi 11,66. Foram prescritos 16 117 medicamentos: 72,69% prescritos pelo nome genérico; 99,39% padronizados pela instituição; 2,86% antimicrobianos; 41,87% injetáveis e 4,39% medicamentos psicotrópicos. Os medicamentos mais prescritos foram: dipirona sódica (7,84%), omeprazol (4,80%) e maleato de enalapril (4,78%). No primeiro nível da classificação do Anatomical Therapeutic Chemical, as classes mais utilizadas foram: sistema cardiovascular (35,18%), trato gastrintestinal (23,42%), sangue e órgãos hematopoiéticos (17,81%). No segundo nível, as classes predominantes foram: antitrombóticos (17,61%),analgésicos (9,30%) e antieméticos (8,20%). Conclusões: Nas prescrições analisadas, o consumo médio de medicamentos foi alto, justificado pela complexidade, idade e doenças concomitantes dos pacientes; parte dos medicamentos foi prescrita pelo nome comercial, contrariando a legislação vigente para o serviço público. Para uma utilização de medicamentos mais racional e segura em idosos no serviço de cardiologia, conclui-se que há necessidade de se estabelecerem padrões mínimos para os indicadores de prescrição referentes ao número de medicamentos, injetáveis, antimicrobianos e psicotrópicos, de acordo com a complexidade do âmbito hospitalar na faixa etária estudada.


Background: Indicators are useful tools for examining drug prescription profiles. Objective: To analyze drug prescription indicators recommended by the World Health Organization in prescriptions for elderly patients in a cardiology ward under Brazil’s Unified National Health System (SUS). Methods: This exploratory descriptive study uses a qualitative approach through analyzing 1,382 prescriptions issued during the hospitalization of 223 elderly patients in the clinical cardiology Ward of a teaching hospital. Results: The average number of drugs per prescription was 11.66, with a total of 16,117 medications prescribed: 72.69% prescribed by their generic names; 99.39% listed as approved by the institution; 2.86% antimicrobials; 41.87% injectables; and 4.39% psychotropics. The medications most frequently prescribed were dipyrone (7.84%), omeprazole (4.80%)and enalapril maleate (4.78%). At the first ATC classification level, the classes most used were: cardiovascular system (35.18%), gastrointestinal tract(23.42%); and blood or blood-forming organs (17.81%). At the second level, the predominant classes were: antithrombotics (17.61%), analgesics (9.30%) and antiemetics (8.20%). Conclusions: In the prescriptions analyzed, average drug consumption was high, justified by thecomplexity, age and concomitant diseases of the patients, with some medications prescribed by brandname, breaching Brazilian National Health System law. This leads to the conclusion that, in order to ensure safer and more rational use of medications for elderly patients in cardiology units, minimum indicator standards must be established for the number of injectable, antimicrobial and psychotropic drugs prescribed, tailored to the complexity of the hospital environment for the age bracket addressed by this study.

Introduction: Preoperatively elevated serum creatinine (SCr) is considered an independent risk factor for morbidity and mortality after cardiac surgery. The aim of this study was to apply the Kidney Disease Improving Global Outcomes classification for acute kidney injury in a population of patients with preoperatively elevated serum creatinine who underwent cardiac surgery (coronary artery bypass grafting or cardiac valve surgery) and to evaluate the acute worsening of renal function as a predictor of 30-day mortality. Methods: This was a single-center retrospective study that included patients from the Postoperative Cardiac Surgery Intensive Care Unit of the Hospital de Base, São José do Rio Preto Medical School. Demographics, type of surgery, laboratory data and pre, peri and postoperative data were obtained from a prospectively collected database. From January 2003 to June 2013, 2,878 patients underwent cardiac surgery, either coronary artery bypass grafting or cardiac valve surgery, at the Hospital de Base of São José do Rio Preto Medical School. Out of those, 918 showed elevated preoperative serum creatinine, with SCr > 1.30 mg/dL for men and > 1.00 mg/dL for women. Five hundred and forty nine patients (60%) undergoing coronary artery bypass grafting and 369 patients (40%) undergoing cardiac valve surgery. A Multivariate Cox Proportional Hazard Model (stepwise) was used to assess the relationship between AKI and mortality at 30 days. Results: Out of the 918 patients studied, 391 (43%) had postoperative AKI: 318 (35%) had Kidney Disease Improving Global Outcomes stage 1, 27 (2.9%) had Kidney Disease Improving Global Outcomes stage 2, and 46 (5.0%) had Kidney Disease Improving Global Outcomes stage 3. Introdução: Creatinina sérica (CSr) elevada no período pré-operatório é considerada um fator de risco independente para morbidade e mortalidade em cirurgia cardíaca. Avaliar o impacto da lesão renal aguda pelos critérios Kidney Disease Improving Global Outcomes como preditor de mortalidade em 30 dias em pacientes submetidos à revascularização miocárdica ou cirurgia valvar com creatinina sérica pré-operatória elevada. Métodos: Este foi um estudo retrospectivo de centro único que incluiu pacientes da Unidade de Cuidados Intensivos em Pós-operatório de Cirurgia Cardíaca do Hospital de Base, Faculdade de Medicina de São José do Rio Preto. Dados demográficos, tipos de cirurgia, dados laboratoriais e informações pré, peri e pós-operatórias foram obtidos a partir de uma coleta prospectiva de banco de dados. Foram considerados 2878 pacientes consecutivamente submetidos à revascularização miocárdica ou cirurgia valvar no período de janeiro de 2003 a junho de 2013. Destes, 918 indivíduos apresentavam creatinina sérica alterada no pré-operatório (CrS > 1.30 mg/dL para homens e > 1.00 mg/dL para mulheres), compreendendo 549 pacientes (60%) submetidos à revascularização miocárdica e 369 pacientes (40%) submetidos à cirurgia valvar. O modelo de riscos proporcionais de Cox foi utilizado para avaliar a relação entre lesão renal aguda e mortalidade em 30 dias. ABSTRACT

Objective: In the clinical scenario of ST-segment elevation acute myocardial infarction, several patients with multivessel coronary atherosclerotic disease are discharged without a defined strategy to monitor the residual atherosclerotic lesions. The clinical endpoints evaluated were cardiovascular death, symptoms of angina pectoris, rehospitalization for a new acute coronary syndrome, and the necessity of reintervention during the two-year follow-up. Methods: This observational, prospective, and historical study included multivessel coronary atherosclerotic disease patients who were admitted to a tertiary care university hospital with ST-segment elevation acute myocardial infarction and underwent primary percutaneous coronary intervention with stent implantation only at the culprit lesion site; these patients were monitored in the outpatient clinic according to two treatments: the Clinical Group - CG (optimized pharmacological therapy associated with counseling for a healthy diet and cardiac rehabilitation) or the Intervention Group - IG (new staged percutaneous coronary intervention or surgical coronary artery bypass graft surgery combined with the previously prescribed treatment). Results: Of 143 patients consecutively admitted with ST-segment elevation acute myocardial infarction, 57 were eligible for the study (CG=44 and IG=13). Regarding the clinical endpoints, the cardiovascular death rate did not differ between the CG and IG. The symptom of angina pectoris and the rehospitalization rate for a new episode of acute coronary syndrome were accentuated in the CG (P=0.020 and P=0.049, respectively) mainly in individuals with evidence of ischemia evidenced by myocardial scintigraphy (P<0.001 and P=0.001, respectively) which culminated in an even greater need for reintervention (P=0.001) in this subgroup of patients. Objetivo: No cenário do Infarto Agudo do Miocárdio com Supradesnível do Segmento ST, diversos pacientes com doença coronária aterosclerótica multiarterial recebem alta hospitalar sem estratégia definida para seguimento de coronariopatia residual. Avaliamos o desfecho composto por morte cardiovascular, sintoma de angina de peito, reinternação por nova síndrome coronária aguda ou necessidade de reintervenção no seguimento de dois anos. Métodos: Estudo observacional, prospectivo, histórico, incluindo portadores de doença coronária aterosclerótica multiarterial admitidos em serviço terciário universitário com Infarto Agudo do Miocárdio com Supradesnível do Segmento ST submetidos à intervenção coronária percutânea primária com implante de stent apenas na lesão culpada e conduzidos ambulatorialmente conforme duas terapêuticas: Grupo Clínico - GC (terapia farmacológica otimizada associada à orientação dietética saudável e reabilitação cardiovascular) ou Grupo Intervenção - GI (nova ICP estadiada ou revascularização miocárdica cirúrgica aliada ao tratamento previamente descrito). Resultados: De 143 pacientes consecutivamente admitidos com Infarto Agudo do Miocárdio com Supradesnível do Segmento ST, 57 foram elegíveis para o estudo (GC=44 e GI=13). Em relação aos desfechos, o GI não diferiu do GC quanto à taxa de óbito cardiovascular. Genetic and environmental factors affect the pathogenesis of Parkinson's disease (PD). Genetic variants of the enzyme glutathione S-transferases (GST) may be related to the disease. This study aimed to evaluate the influence of genetic variants of GST (GSTT1/GSTM1) and their association with the exposure to environmental toxins in PD patients. We studied 254 patients with PD and 169 controls. The GSTM1/GSTT1 variants were analyzed by polymerase chain reaction. We applied the Fisher's exact test and the χ2 test for statistical analysis (p<0.05). The present and absence for GSTT1 and GSTM1 were similar in patients and controls. The null for GSTT1 and GSTM1 (0/0) and exposure to pesticides prevailed in patients (18%) compared to controls (13%, p=0.014). This study suggests the association between PD and previous exposure to pesticides, whose effect may be enhanced in combination with null for GSTT1/GSTM1.


Fatores genéticos e ambientais influenciam a patogênese da doença de Parkinson (DP). Variantes genéticas das enzimas glutationa S-transferases (GST) parecem estar envolvidas com a doença. Os objetivos deste estudo foram avaliar a influência de variantes genéticas de GST (GSTT1/GSTM1) e sua associação com exposição a toxinas ambientais em pacientes com DP. Foram estudados 254 pacientes com DP e 169 controles. As variantes para GSTM1/GSTT1 foram analisadas por reação em cadeia da polimerase. Para análise estatística foram aplicados os testes de Fisher e do χ2 (p<0,05). Tanto a presença quanto a nulidade para GSTT1 e GSTM1 foram semelhantes em pacientes e controles. A nulidade para GSTT1 e GSTM1 (0/0) e contato com agrotóxicos prevaleceu nos pacientes (18%) em relação aos controles (13%, p=0,014). Este estudo sugere associação entre DP e contato prévio com agrotóxicos, cujo efeito parece potencializado em combinação com nulidade para GSTT1/GSTM1.

OBJECTIVE: We evaluated patients underwent cardiac valve surgery in the presence of infective endocarditis in an attempt to identify independent predictors of 30-day mortality. METHODS: We evaluated 837 consecutive patients underwent cardiac valve surgery from January 2003 to May 2010 in a tertiary hospital in São José do Rio Preto, São Paulo (SP), Brazil. The study group comprised patients who underwent intervention in the presence of infective endocarditis and was compared to the control group (without infective endocarditis), evaluating perioperative clinical outcomes and 30-day all cause mortality. RESULTS: In our series, 64 patients (8%) underwent cardiac valve surgery in the presence of infective endocarditis, and 37.5% of them had surgical intervention in multiple valves. The study group had prolonged ICU length of stay (16%), greater need for dialysis (9%) and higher 30-day mortality (17%) compared to the control group (7%, P=0.020; 2%, P=0.002 and 9%, P=0.038; respectively). In a Cox regression analysis, age (P = 0.007), acute kidney injury (P = 0.004), dialysis (P = 0.026), redo surgery (P = 0.026), re-exploration for bleeding (P = 0.013), tracheal reintubation (P <0.001) and type I neurological injury (P <0.001) were identified as independent predictors for death. Although the manifestation of infective endocarditis influenced on mortality in univariate analysis, multivariate Cox regression analysis did not confirm such variable as an independent predictor of death. CONCLUSION: Age and perioperative complications stand out as predictors of hospital mortality in Brazilian population. Cardiac valve surgery in the presence of active infective endocarditis was not confirmed itself as an independent predictor of 30-day mortality.


OBJETIVO: Avaliamos pacientes submetidos à cirurgia valvar em vigência de endocardite infecciosa na tentativa de identificar preditores independentes de mortalidade intrahospitalar em 30 dias. MÉTODOS: Foram avaliados 837 pacientes consecutivamente submetidos à cirurgia valvar, no período de janeiro de 2003 a maio de 2010, em um hospital terciário de São José do Rio Preto, SP, Brasil. O Grupo de Estudo compreendeu indivíduos submetidos à intervenção em vigência de endocardite infecciosa e foi comparado ao Grupo Controle, considerando complicações clínicas perioperatórias e óbito por todas as causas em 30 dias. RESULTADOS: Em nossa casuística, 64 (8%) pacientes foram submetidos à cirurgia valvar em vigência de endocardite infecciosa, sendo 37,5% deles com indicação de intervenção cirúrgica em múltiplas valvas. O Grupo de Estudo apresentou maior permanência em Unidade de Terapia Intensiva (16%), necessidade de diálise (9%) e maior mortalidade em 30 dias (17%) comparado ao Grupo Controle (7%, P=0,020; 2%, P=0,002 e 9%, P=0,038; respectivamente). A análise de regressão de Cox confirmou idade (P=0,007), lesão renal aguda (P=0,004), diálise (P=0,026), reoperação (P=0,026), reintervenção por sangramento (P=0,013), reintubação orotraqueal (P<0,001) e lesão neurológica tipo I (P<0,001) como preditores independentes para óbito. Embora a manifestação de endocardite infecciosa influencie na mortalidade na análise univariada, a regressão de Cox não confirmou tal variável como preditor independente de óbito em nossa casuística. CONCLUSÃO: Idade e complicações perioperatórias destacam-se como preditores de mortalidade hospitalar em população brasileira. Cirurgia valvar em vigência de infecção ativa não se confirma como preditor independente de óbito nesta casuística.

FUNDAMENTO: As discrepâncias entre os diagnósticos clínicos e em autópsia persistem em todo o mundo. OBJETIVO: Avaliamos as autópsias em um hospital-escola para analisar a precisão dos diagnósticos cardiovasculares clínicos em comparação aos achados post-mortem. MÉTODOS: As 409 autópsias consecutivas entre 2003 e 2006 foram analisadas em um hospital terciário de São José do Rio Preto, São Paulo (SP), Brasil. A comparação dos achados cardiovasculares clínicos e patológicos foi realizada por meio da classificação de discrepâncias de Goldman. RESULTADOS: A taxa de autópsia no hospital foi de 8 por cento. As causas cardiovasculares de óbito representavam 42,8 por cento (175 de 409 pacientes) dos diagnósticos de autópsia. Em 98 pacientes (56 por cento), houve discrepâncias significativas (classes I e II), o que representa uma grande proporção de diagnósticos equivocados de infarto mesentérico (84,6 por cento), infarto agudo do miocárdio (64,7 por cento), dissecção da aorta (64,2 por cento) e embolia pulmonar (62,5 por cento). Foram observadas maiores taxas de concordância para a insuficiência cardíaca congestiva (59 por cento) e para o acidente vascular cerebral isquêmico agudo (58,8 por cento). A idade, o sexo, o tempo de permanência e a última unidade de admissão no hospital não foram associados aos critérios de Goldman. CONCLUSÃO: As discrepâncias dos diagnósticos clínicos e em autópsia relativos à morte cardiovascular permanecem elevados no Brasil, a despeito dos recursos tecnológicos disponíveis. Além disso, nossos achados reforçam a importância do exame post-mortem como uma contribuição para a melhoria da assistência médica.


BACKGROUND: Discrepancies between clinical and autopsy diagnoses persists worldwide. OBJECTIVE: We evaluated autopsies in a university hospital in order to assess the accuracy of clinical cardiovascular diagnosis compared to postmortem findings. METHODS: Four hundred nine consecutive autopsies between 2003 and 2006 were analyzed in a tertiary-care hospital in São José do Rio Preto, SP, Brazil. The comparison of clinic-pathological cardiovascular findings was performed using Goldman's discrepancies classification. RESULTS: Autopsy rate at the hospital was 8 percent. Cardiovascular causes of death represented 42.8 percent (175 out of 409 patients) of autopsy diagnoses. In 98 (56 percent) patients, there were major discrepancies (class I and II), representing a large proportion of misdiagnoses for mesenteric infarction (84.6 percent), acute myocardial infarction (64.7 percent), aorta dissection (64.2 percent), and pulmonary embolism (62.5 percent). Highest concordance rates were observed in congestive heart failure (59 percent) and acute ischemic stroke (58.8 percent). Age, sex, length of stay and the last admission unit at the hospital were not associated with Goldman criteria. CONCLUSION: Clinic-autopsy discrepancies concerning cardiovascular death remain high in Brazil, despite technological resources available. Moreover, our findings reinforce the importance of postmortem examination in contributing to medical care improvement.

ABSTRACT

The influence of medical students' knowledge concerning end-of-life care, considering ethical theories and clinical practice, remains controversial. We aimed to investigate medical students' knowledge of bioethical concepts related to moral kinds of death (euthanasia, disthanasia, and orthothanasia) and to analyze the influence of their clinical experience on practicing such approaches in a tertiary hospital in the state of São Paulo, Brazil. We interviewed 180 medical students [distributed in Group 1 (G1) - first to third- year students, and Group 2 (G2) - fourth to sixth-year students] to evaluate the influence of the course on "medical ethics" on ethical theories and clinical practice, using a closed questionnaire. The course on "medical ethics" did not distinguish the groups (P=0.704) in relation to bioethical concepts. Neologisms such as "cacothanasia" and "idiothanasia" were incorrectly viewed as bioethical concepts by 28 percent of the interviewees. Moreover, 45.3 percent of the sample considered health care professionals incapable of managing terminally ill patients, especially G2 (29 percent) as compared to G1 (16.5 percent, P=0.031). The concept of euthanasia was accepted by 41 percent of sample, as compared to 98.2 percent for orthothanasia. Among medical students that accepted ways to abbreviate life (22.9 percent), 30.1 percent belonged to G1, and only 16.1 percent to G2 (P=0.049). These medical students were unfamiliar with common bioethical concepts. Moreover, they considered healthcare professionals incapable of managing terminally ill patients. The ethical ideal of the "good death" reflects better acceptance of orthothanasia by medical students, suggesting a tendency to apply it in their future clinical practice.


A influência do conhecimento de estudantes de medicina sobre o manejo com o fim da vida, considerando teorias éticas e aplicabilidade clínica, permanece controversa. Objetivamos investigar o conhecimento de estudantes de medicina sobre conceitos bioéticos a respeito dos tipos morais de morte (Eutanásia, Distanásia e Ortotanásia) e analisar a influência de suas experiências clínicas para praticá-los, em um hospital terciário do estado de São Paulo, Brasil. Nós entrevistamos 180 estudantes de medicina [distribuídos em Grupo 1 (G1) - estudantes do primeiro ao terceiro ano de graduação em medicina e Grupo 2 (G2) - estudantes do quarto ao sexto ano] para avaliar a influência do "curso de ética médica" em teorias éticas e prática clínica, utilizando um questionário fechado. O "curso de ética médica" não diferiu os grupos (P=0.704) sobre os conceitos bioéticos. Neologismos como Cacotanásia e Idiotanásia foram erroneamente considerados como conceitos bioéticos por 28 por cento dos indivíduos. Além disso, 45.3 por cento da amostra consideraram os profissionais de saúde inaptos ao manejo de pacientes terminais, principalmente G2 (29 por cento) comparado a G1 (16.5 por cento, P=0.031). O conceito de Eutanásia foi aceito por 41 por cento da amostra, enquanto Ortotanásia por 98.2 por cento. Dentre os estudantes de medicina que aceitaram maneiras para abreviar a vida (22.9 por cento), 30.1 por cento deles pertenciam a G1 enquanto somente 16.1 por cento a G2 (P=0.049). Estudantes de medicina mostram desconhecimento sobre conceitos clássicos em Bioética. Além disso, consideram os profissionais de saúde inaptos ao manejo de pacientes terminais. O ideal ético considerando a "boa morte" reflete melhor aceitação dos estudantes de medicina pela Ortotanásia, sugerindo tendência em aplicá-la em sua futura prática clínica.

OBJECTIVE: To evaluate whether lipid profile (LP), apolipoprotein A-1 (apo A-I) and malondialdehyde (MDA) have any relationship with physical exercise by comparing the groups of footballers (FG) with sedentary individuals (CG) and their relatives (RFG and RCG). SUBJECTS AND METHODS: Twenty individuals from FG and CG, 60 from RFG, and 57 from RCG were studied. RESULTS: FG showed lower levels of total cholesterol (119.5 ± 37.9 mg/dL), LDL-cholesterol fraction (53.6 ± 30.3), apo A-I (116.7 ± 11.9), and higher level of HDL-cholesterol fraction (HDLc) (49.7 ± 8.5) compared to RFG (148.3 ± 36.9, P = 0.02; 82.4 ± 37.7, P < 0.01; 124.6 ± 10.2, P = 0.03; and 42.7 ± 7.7, P < 0.01; respectively). Moreover, FG had reduced levels of MDA (101.0 ± 77.0 ng/mL) compared to CG (290.0 ± 341.0, P = 0.03) and RFG (209.9 ± 197.5, P = 0.04). CONCLUSIONS: These results suggest an association between physical exercise and lower levels of MDA in FG. Physical activity seems to promote beneficial effects on the LP regardless of the genetic influence considering HDLc levels.

SUJEITOS E MÉTODOS: Foram avaliados 20 indivíduos de FG e CG, além de 60 familiares de FG (RFG) e 57 de CG (RCG). RESULTADOS: FG mostrou menores níveis (média ± DP [mg/dL]) de colesterol total (119.5 ± 37.9), LDL colesterol (53.6 ± 30.3), e apo A-I (116.7 ± 11.9), e maiores níveis de HDL colesterol (HDLc) (49.7 ± 8.5) comparado ao RFG (148.3 ± 36.9, P = 0.02; 82.4 ± 37.7, P < 0.01; 124.6 ± 10.2, P = 0.03; e 42.7 ± 7.7, P < 0.01; respectivamente). Além disso, o FG mostrou níveis reduzidos de MDA (101.0 ± 77.0 [ng/mL]) comparado a CG (290.0 ± 341.0, P = 0.03) e RFG (209.9 ± 197.5, P = 0.04). CONCLUSÕES: Esses resultados sugerem que existe uma associação entre atividade física e níveis reduzidos de MDA em FG. O exercício físico parece promover efeitos benéficos no PL independente da influência genética considerando os níveis de HDLc.

ABSTRACT

Objetivos. Analisar perfil bioquímico e caracterizar síndrome metabólica (SM) em pacientes com acompanhamento cardiológico, conforme critérios de NCEP-ATPIII e IDF. MÉTODOS: Foram estudados 200 pacientes e 140 controles, considerando colesterol total (CT), fração de colesterol de lipoproteína de alta (HDLc), baixa (LDLc) e muito baixa densidade (VLDLc), triglicérides (TG), glicemia de jejum, cintura abdominal e hipertensão arterial sistêmica (HAS). Admitiu-se nível de significância P<0,05. RESULTADOS: Pacientes apresentaram níveis elevados de glicemia (103±31,4mg/dL) e reduzidos de HDLc (48±13,4mg/dL) comparado aos controles (88±29,7mg/dL, P<0,0001; 53±15,9mg/dL, P=0,0075; respectivamente). Controles do sexo masculino, entre 31 e 50 anos, mostraram níveis aumentados de CT (215±40,4mg/dL), LDLc (134±34mg/dL), VLDLc (30±11,8mg/dL) e TG (150±59,4mg/dL) comparado às mulheres (185±38,2mg/dL, P=0,0137; 111±35,8mg/dL; P=0,0324; 19±9,7mg/dL; P=0,0009; 93±49mg/dL, P=0,0010; respectivamente). Mulheres acima dos 50 anos mostraram aumento de CT (216±35,9mg/dL), HDLc (54±12,8mg/dL) e LDLc (138±30,8mg/dL) comparado aos homens (190±44,7mg/dL, P=0,0103; 47±14,5mg/dL, P=0,0229; 119±33,3mg/dL; P=0,0176; respectivamente). Os critérios NCEP-ATPIII e IDF caracterizaram SM em 35,5 por cento e 46 por cento dos pacientes, respectivamente, com destaque para glicemia, TG e HAS. CONCLUSÃO: Níveis elevados de glicemia e reduzidos de HDLc destacam-se nos pacientes. Perfil lipídico alterado, em homens entre 31 e 50 anos, confere maior risco para doenças cardiovasculares em adulto jovem, enquanto padrão semelhante nas mulheres, após essa faixa etária, pode refletir alterações hormonais fisiológicas. Ambos os critérios para diagnóstico de SM distinguem pacientes de controles, destacando-se IDF, com parâmetros, no entanto, nem sempre associados a alto risco. OBJECTIVE: To analyze the biochemical profile and to characterize metabolic syndrome (MS) in patients with cardiologic medical assistance using NCEP-ATPIII and IDF definitions. METHODS: Two hundred patients and 140 controls were studied, considering total cholesterol (TC), HDL-cholesterol (HDLc), LDL-cholesterol (LDLc), VLDL-cholesterol (VLDLc), triglycerides (TG), fasting glycemia, abdominal waist and hypertension. Significance level was defined as P<0.05. RESULTS: Patients showed increased glycemia levels (103±31.4mg/dL) and reduced HDLc levels (48±13.4mg/dL) when compared to controls (88±29.7mg/dL, P<0.0001 and 53±15.9mg/dL, P=0.0075; respectively). Male controls 31-50 years old showed increased TC levels (215±40.4mg/dL), LDL-cholesterol (134±34mg/dL), VLDL-cholesterol (30±11.8mg/dL) and TG (150±59.4mg/dL) when compared to women (185±38.2mg/dL, P=0.0137; 111±35.8mg/dL; P=0.0324; 19±9.7mg/dL; P=0.0009; 93±49mg/dL, P=0.0010; respectively). Women over 50 years of age showed increased TC concentrations (216±35.9mg/dL), HDL-cholesterol (54±12.8mg/dL) and LDL-cholesterol (138±30.8mg/dL) when compared to men (190±44.7mg/dL, P=0.0103; 47±14.5mg/dL, P=0.0229; 119±33.3mg/dL; P=0.0176; respectively). NCEP-ATPIII and IDF definitions had characterized MS in 35.5 percent and 46 percent of patients, respectively, bolding glycemia, TG and hypertension. CONCLUSION: Elevated glycemia levels and reduced HDLc levels were detected in patients. Altered lipid profile observed in men 31-50 years old signals higher risk for cardiovascular diseases in young adults, while a similar profile in aged women can reflect hormonal physiological changes. Both definitions for MS diagnosis discriminate patients from controls, especially IDF, sometimes with lower capacity to determine high risk for cardiovascular complications.
